چکیده

This retrospective study compared mid-term outcome of a simple crural bypass (SCB, n=72) and branched crural bypass (BCB, n=23) constructed with reverse or in situ saphenous grafts or prosthetic grafts in patients with peripheral arterial disease grade III to IV (Fontaine classification) and type D lesions (TASC II classification). Life table analysis showed comparable primary (P=0.83), assisted (P=0.76), and secondary patency rates (P=0.78), limb salvage rates (P=0.64), and survival rates (P=0.81) between BCB and SCB. Comparison between the saphenous vein grafts and prosthetic grafts showed better primary (P=0.0089), assisted (P=0.0013), and secondary patency rates (P=0.0089) where venous grafts were used, but not limb salvage rates (P=0.072). In the SCB group, anastomoses to the anterior tibial artery had worse primary (P=0.023), assisted (P=0.035), and secondary (P=0.042) patency rates, but comparable limb salvage rates (P=0.57). We conclude that BCB provides no advantage but also no additional risk for the patient. When choosing a target crural artery, evidence shows the posterior tibial artery or fibular artery should be preferred to the anterior tibial artery.

Chronic critical limb ischemia.

BACKGROUND Some 40 000 lower limb amputations are performed in Germany each year, 70% of them in diabetics. About 80% of all major amputations may be preventable with the use of new interventional and vascular surgical procedures, particularly on the arteries of the leg and foot. We present the current state of the art in revascularization techniques and evaluate their usefulness for preservati...

The extent of atherosclerotic lesions in crural arteries predicts survival of patients with lower limb peripheral artery disease: A new classification of crural atherosclerosis.

BACKGROUND AND AIMS Several studies report correlation of ankle brachial index (ABI) values and mortality. However, no studies exist on the predictive value of anatomical distribution of atherosclerotic lesions and the extent of atherosclerosis at defined arterial segments on life expectancy. The aim of the present study was to evaluate the significance of both extent and localisation of athero...

Intraoperative evaluation of revascularization effect on ischemic muscle hemodynamics using near-infrared diffuse optical spectroscopies.

Arterial revascularization in patients with peripheral arterial disease (PAD) reestablishes large arterial blood supply to the ischemic muscles in lower extremities via bypass grafts or percutaneous transluminal angioplasty (PTA). Currently no gold standard is available for assessment of revascularization effects in lower extremity muscles. This study tests a novel near-infrared diffuse correla...

Hyperhomocysteinaemia is not associated with isolated crural arterial occlusive disease: The Hoorn Study.

OBJECTIVES Hyperhomocysteinaemia is an independent risk factor for peripheral arterial disease (PAD). The localization of peripheral arterial disease is clinically relevant, because proximal (aortoiliac and femoropopliteal) disease is associated with a particularly poor overall prognosis, whereas isolated distal (i.e. crural) disease is associated with a better overall prognosis. The aim of the...
